Office Policies

To provide the best possible service and availability to our patients, it is our policy to charge a fee of $50 for office visit appointments and $100 for scheduled surgical appointments not canceled or canceled at least 24 hours before appointment time. If you are having a cosmetic procedure, it is our policy to charge a deposit if it's not canceled 24 hours prior. Please call us as early as possible if you need to reschedule your appointment.
All minors must be accompanied by a parent or legal guardian at the first visit with proper identification. The parent or legal guardian must sign a treatment to minor consent for the minor to be treated at his/her next appointment in parent or legal guardian's absence.
Please allow 24 hours for all refill requests.

Insurance
Medical and surgical visits will be filed with your insurance carrier. It is the responsibility of the patient to provide accurate insurance and personal information and to verify network status prior to your visit. If your insurance requires a referral, it is your responsibility to provide the referral prior to your visit.
Per your contract with your insurance company, a co-payment, co-insurance, or deductible may be due. These are calculated based on our best efforts, and any differences will be billed or credited to you. You will be responsible at the time of service for the payment of such copays or deductibles, as well as any past due balances.
Recent changes in healthcare markets have altered insurance coverages to shift more of the cost of care to our patients. Many policies have higher deductibles which means, even if a procedure is covered by insurance, you may be responsible for the majority or entirety of the visit charges until your deductible is met.
Patient Payments
You should expect to make some payments either before your office visit or at the time services are rendered.
Allowable Forms of Payment
We accept the following forms of payment for all fees.

· Credit Card – Visa, MasterCard, Discover, Debit Card, Care Credit
Returned Check Fees
If your check payment is returned you will be assessed a returned check fee of $35 in addition to the office service fee.
Late Arrival Policy
As a courtesy to our other patients, any patient who is more than 15 minutes late for his/her appointment will not be seen, may be rescheduled and will be treated as a “late cancellation”. Please arrive for your appointment 15 minutes early to provide current insurance information, and if necessary sign consents and fill out any required office paperwork.
Product Return Policy
In the event that you are not satisfied, we will gladly provide an in-office credit or exchange if returned unopened within 30 days of your purchase date. NO REFUNDS ON PRESCRIPTIONS!
Gift Cards may only be redeemed for products or services. Cannot be redeemed for cash or credit. Gift Card sales are final and non-refundable.
Accepted Insurance Carriers
We accept Medicare and most traditional insurance plans. Please get in touch with our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We always recommend contacting your insurance carrier to confirm our providers are in your network before your visit.
